San Juan Health Partnership for the Homeless
San Juan, PR
The San Juan Health Partnership for the Homeless will deliver affordable, accessible quality health care for people without homes or at risk of becoming homeless. Outreach, case management, preventive and primary care, mental health counseling, social services and addiction treatment will all be offered at one site. The sponsoring organization, La Fondita de Jesus, is a well-established service provider incorporating a soup kitchen, transitional housing and other programs such as behavioral health care. The project has garnered impressive support of local physician leadership including the Puerto Rican Medical Society and the medical school of the University of Puerto Rico. A key component of the new program will be patient navigation to enable clients from often-excluded populations to avail themselves of mainstream systems of health services.

Chronic Disease Prevention and Control Project

Using peer health promoters to reach patients with asthma, diabetes and hypertension, this project focuses on changing from provider-centered care to patient-centered care in a rural area of Puerto Rico. They also organize community-based preventive health programming, stressing healthy behaviors and physical activity for everyone from homebound seniors to children at school.

Health Youth Leadership Program For Puerto Rico

Five Puerto Rican community-based organizations have joined in a partnership to effect change in the way community-based youth leadership is exerted in Puerto Rico, and to effectively apply it to health promotion and other related outcomes. A Health Youth Leadership Program for Puerto Rico will encourage access to basic health care, promote health and reduce the personal, social and economic harm caused by substance abuse, alcohol and illegal drugs.
