Off the Streets

Off the Streets helps women involved in prostitution move toward safety, recovery, empowerment and community reintegration. Women with histories of prostitution collaborated with police and court leadership to develop this comprehensive intervention. It blends peer support with existing housing, substance abuse treatment, employment training, primary care and mental health services. Women also participate in alternative therapies such as yoga, massage and journal writing. This public/private partnership operates at the intersection of the criminal justice system with healthcare, substance abuse treatment and community development.

Community Advocacy Program (CAP) - A Medical-Legal Collaborative
Cleveland, OH
Based on a medical-legal advocacy program piloted in a children's hospital, this project moves attorneys out to three community health centers in Cleveland and expands services from vulnerable children to include adults, the elderly, Spanish-speaking people and individuals newly released from incarceration. The funding partnership includes large law firms whose partners understand the impact legal issues such as adequate housing, heat and lead removal can have on health of families facing significant poverty and health care disparities.

Creando Posibilidades/Creating Possibilities
Cleveland, OH
Establish a health care careers center at West Side Ecumenical Ministry, in partnership with hospitals, educational institutions and others, to increase the number of Hispanic Clevelanders successfully entering health careers. This multi-pronged workforce initiative recruits Spanish-speaking secondary students and adults to train as nurse's aides and Licensed Practical Nurses. The individual trainees move out of unemployment and poverty to jobs with health insurance, tuition benefits and an upwardly mobile career path. These bilingual professionals will help reduce cultural barriers throughout the health care system.

Love, Faith and Family - Amish Genetic Disease Education and Care
Middlefield, OH
Although genetic and metabolic diseases are rare in the general population, there is a rapidly increasing incidence among Amish families. Das Deutch Center for Special Needs Children offers early diagnosis and treatment that can greatly improve the quality of life for these vulnerable children and their families. This project will expand early diagnosis and treatment services in the Amish Geauga settlement and offer services for Amish families in the Holmes County settlement. A culturally appropriate outreach program will be developed and implemented to educate parents about genetic diseases and the value of newborn screening.

Children Who Witness Violence Program
Akron, OH
Program will provide early intervention to children who witness repetitive violence and instability in the home. The trauma of witnessing violence places children at high risk of depression, anxiety, and anger. The proposed intervention begins with police while investigating a domestic violence incident, and call-in referral of child witnesses to a 24 hour line. A team of two home visitors will respond within 30 minutes on site to focus on resolution of the crisis. Follow-up for the family and child will be provided. Subsequent home based interventions will assess needs and transition the family/child(ren) to ongoing trauma support services.
