PCHC's Community Clinic Project
Bangor, ME

To meet the mental health and substance abuse treatment needs of people at risk of losing their housing and of those already without homes, this Penobscot Community Health Center will assemble a new team of psychiatric and medical professionals to bring these services directly to three shelters and a soup kitchen. Knowing one's housing is tenuous or living without a home can create serious psychological pressure, yet many struggling families cannot easily find or afford behavioral health services or substance abuse treatment when needed. The project staff will use an evidence-based framework to integrate medical and mental health care and employ electronic record keeping to coordinate care for this transient population. Another goal of the project is to capture their experience and utilize a thorough evaluation to enable other community health centers across the country to replicate the program.

Portland Identification and Early Referral Project
Portland, ME

This project aims to reduce the incidence of psychosis and to prevent disability from psychotic disorders among young people in Greater Portland, Maine. A team of specialized mental health clinicians work with primary care physicians and educational counselors to refer young persons at risk and provides treatment in close partnership with the family, while the project uses intensive commercial media to educate and inform the public.

The program's decentralized care management component promotes wellness through the use of a Personal Advocacy system that provides education, guidance, support, and assistance to all members.

Healthy Futures Community Primary Care Project
Winthrop, ME

Health Advocates, community health nurses, wil imporve access to primary and preventative care for the 16,000 residents of six small Maine towns. Through this, it is anticipated the health status will be improved and medical expenses will be reduced. The Health Advocates will also work with town residents to develop health projects and influence policies impacting on health.
