Temporary LockDown

Building on the success of Storycatcher Theatre's program for incarcerated girls, the Illinois Department of Juvenile Justice requested a new gender-specific program to serve boys at the Illinois Youth Center in Chicago. This juvenile prison is the last stop before the boys return home and a critical juncture in preparing to make positive life choices. Through creating original musical theatre productions, Temporary LockDown helps incarcerated adolescents understand and reduce violent behaviors. The theatre project increases self-esteem; develops communications, conflict resolution and leadership skills; and serves as a gateway to therapy.

Vision of Hope Health Alliance
Chicago, IL

Uninsured patients may delay primary care appointments, but will seek medical attention when vision problems interfere with employment or driving. Doctors at the Illinois College of Optometry observed that many of the eye conditions treated at their community clinics were the result of chronic diseases such as hypertension and diabetes. Using a case manager model, referrals from the eye clinics will be made to 17 partner organizations including federally qualified health centers, homeless service agencies, addiction treatment services and YMCAs. These community agencies will also refer clients to Vision of Hope for comprehensive eye exams. Optometrists will be trained to screen patients for underlying medical problems and optometry students will be encouraged to continue providing vision care as a community service after they graduate.

Doorway to Integrated Health Care

This initiative seeks to improve health outcomes for people with severe and persistent mental illness by providing individualized primary and preventive care from advanced practice nurses. It expands a successful pilot program conducted by Thresholds, a recognized provider of psychiatric rehabilitation services to low income and uninsured individuals, and the University of Illinois at Chicago College of Nursing. These partners are committed to helping Thresholds' members manage chronic diseases such as diabetes, pursue behavioral health goals such as smoking cessation, counter the unhealthful side effects of some pharmaceutical agents and reduce reliance on emergency rooms.

The Chicago Project for Violence Prevention is a collaborative public health approach to reducing violence that uses outreach, clergy, public education, and community mobilization to change norms and provide on-the-spot alternatives for highest risk persons. Twelve high-risk communities in Chicago have shown between 45% and 92% reductions in shootings using this approach.

Chicago Health Connection Doula Project

A collaborative project providing birthing support to pregnant teens in low income communities. The project trains and employs lay women as doulas or birthing assistants to develop relationships that provide information, advocacy, and physical and emotional support to birthing teens during the prenatal, intrapartum and postpartum periods.
