Transition from Jail to Community - Palm Beach
West Palm Beach, FL

Using an individualized approach to frequent re-offenders who cycle in and out of the county jail, re-entry coaches will work with clients during their incarceration to develop detailed assessment and discharge plans. Upon release, clients will be directly connected to housing and needed services such as job training and employment, primary and behavioral health services, substance abuse treatment, and education. More than 25 community agencies actively participated in a local funder-led planning process to link separate services into a supportive wraparound system to reduce recidivism.

Girl Matters: It's Elementary!

Girl Matters: It's Elementary! offers a gender-responsive intervention program to effectively address the underlying issues contributing to young girls' (grades K-5) disruptive behaviors in two Duval County, FL schools. The main goal is to reduce elementary school suspensions and expulsions and prevent the arrests and incarcerations that send girls into the juvenile justice system. The comprehensive approach includes training for school staff and three tiers of service: short-term in-school interventions; work with a mentor and community referral networks; and intensive care management, family counseling and in-home support.

Seniors on a Mission

Seniors on a Mission engages live-alone senior citizens in meaningful, team-oriented volunteerism at local nonprofit organizations. The project reduces the social isolation that can lead to depression and medical problems in older persons by providing safe, convenient transportation to accessible, comfortable places to work; a nutritious meal; and coordination with the needs of area nonprofits. The seniors' pro bono services have been enthusiastically received for projects such as bar-coding new elementary textbooks, labeling thousands of fundraising letters, and repairing items donated to a clothing bank.

Kinship Cares

Kinship care is the fastest growing form of home placement for children whose parents cannot care for them due to illness, incarceration or family social factors. Informal custodial arrangements between family members keep children connected to their kin and community, but they often do so without the financial, legal and health benefits available within the foster care system. This partnership between a community social service agency, legal aid and a county health system employs nurses as health navigators to assure that the health care needs of both children and their adult caregivers are addressed.

Caregiving Youth Project

Caregiving youth bear significant responsibility for parents, grandparents or siblings who are ill or disabled. These youth are often isolated, unable to participate in age-appropriate activities and do poorly in school. The Caregiving Youth Project will identify middle-school students most in need of support and provide in-school counseling, respite care, outside activities and links to existing services such as transportation, medical care and equipment, food services and tutoring. Local funders are highly engaged in the development of the program, which is modeled on best practices in the United Kingdom and Australia.

EXTREME Health Challenge

Building on 20 years of experience creating live puppet performances in schools, the ''EXTREME Health Challenge'' uses an interactive game show format to promote healthy nutrition and exercise for children grades K-5 in Florida schools. The script follows the mandated Sunshine State Curriculum and teachers receive grade-appropriate classroom lesson plans that can be used before and after performances. The auditorium-based show will reach more than 200,000 children over the life of the grant. As it travels to approximately 360 schools throughout the state, it will reinforce other childhood obesity prevention activities including those already funded through the LIFP program.

Targeting all fourth and fifth graders at 50 South Florida elementary schools, this bi-county physical activity program seeks to combat childhood obesity and promote lifelong wellness emphasizing fitness and good nutrition. Local funders convened this regional collaboration of schools, hospitals, community organizations, and county health, parks and recreation departments. The Schools of Wellness project will engage children in three complementary and well-tested program components that encourage walking and active lifestyles. Simultaneously a social marketing campaign will raise community-wide awareness to foster an environment where children and families engage in physical activities.
