Reporting Guidelines

Reporting Requirements

Unless otherwise instructed, ALL grantee reports should be submitted by e-mail to the Local Funding Partnerships office using grantreports@localfundingpartnerships.org.

  • Annual Reporting Guidelines for annual reports due one month after the grant's yearly end date.
    • Annual Narrative - describing activites for the 12-month period
      • New for your 2009 Annual Narrative - We recognize that the economic downturn may have impacted your organization’s ability to carry out the RWJF-funded project. Has your ability to secure local match been made more difficult?  Have there been changes in the structure or staffing of your project or organization? Are there changes in demand for services or the capacity of your agency to provide services? Please address this issue in response to question number 4 where we ask if there is anything else you want to tell RWJF.
    • Financial - covering expenditures for the last six-month perior of each grant year.
      • Please name your file using this format: Grant#AFRYr (see Report Descriptions and Naming Standards below))
    • Match Confirmation form that grantees use annually to confirm information on all funders providing the dollar-for-dollar match for the coming year.
  • Final Reporting Guidelines
  • Interim Six-month Reporting Guidelines include both narrative and financial requirements when reporting your progress for the first six-month period of each grant year.

  • Report Descriptions and Naming Standards
    • E-mail electronc versions of reports , bibliographies and products to grantreports@localfundingpartnerships.org should use naming standards set forth here.
