Reporting Requirements
IMPORTANT NOTE: Unless otherwise instructed, ALL grantee reports should be submitted by e-mail to the Local Funding Partnerships office using grantreports@localfundingpartnerships.org and NOT to the @rwjf.org address listed in the linked documents.
- Grantee Reporting Instructions - overview of reporting requirements
- Annual Reporting Guidelines for annual reports due one month after the grant's yearly end date.
- Annual Narrative - describing activites for the 12-month period
- Financial - covering expenditures for the last six-month perior of each grant year.
- Please name your file using this format: Grant#AFRYr (see Report Descriptions and Naming Standards below))
- Match Confirmation form that grantees use annually to confirm information on all funders providing the dollar-for-dollar match for the coming year.

- Final Reporting Guidelines
- Final Narrative - filed at the end of your grant; due 30 days after the end-date for your grant
- Financial - covering activities of the last six-month period of your grant
- Bibliography - complete listing of materials from your project submitted with your final report

- Interim Six-month Reporting Guidelines include both narrative and financial requirements when reporting your progress for the first six-month period of each grant year.
